I've been waiting for someone to start a post about this!
I don't think the young lady in waiting is Shallan, because in interviews he has stated that she is a relativly new character, from the latest version.
Especially the part about ten protectors of mankind (Each driven mad in a slightly different way) excited me when I read this the summer of 2009.

Merrin was Kaladins original name, and was both to feminine and to alike to Perrin. But, they probably are unrelated.
