Wow.  Why do they have a policy for strange situations if they wont even follow it?  It's like having lifeboats and jumping into shark infested waters.  While bleeding because you are missing a chunk of thumb.

They have a policy to avoid such debaucles. Unfortunately people read the policies and think, "that will never happen", then promptly forget about it.

no, just efficient. How much effort can/should you donate to remembering what to do in the case of an event that has a record of not occurring and which you dont' ever expect to happen? not much. there are better, more productive things to remember.
